Conservative treatment patterns in patients with neck and back pain prior to multidisciplinary spine clinic consultation: a mixed methods study from a Danish hospital

Abstract

PurposeTo describe treatments and care pathways of patients with neck and back pain (NBP) before the first consultation at a multidisciplinary spine clinic of Zealand University Hospital, Denmark. Secondary aims were to describe: 1) the impact of NBP on daily life, and 2) the impact of comorbidities on NBP.MethodsCross-sectional mixed methods study using a survey with closed- and open-ended questions. Open-ended questions formed the basis for short qualitative interviews. Data was collected from 112 patients after the first consultation. Quantitative data were analysed descriptively, and thematic analysis was used for qualitative data. Qualitative data were embedded within the primarily quantitative framework.ResultsCommon treatments included physiotherapy (87.5%), medication (79.5%), massage (49.1%), and chiropractic care (40.2%). Many had consulted multiple providers and tried alternative treatments. Overall, 66.1% rated their treatment and referral experience as good or very good. Experiences with waiting time and continuity were mixed. NBP significantly affected daily activities and work in 54.4% of patients. Comorbidities, present in 30.4%, were perceived to worsen NBP.ConclusionPatients had tried various treatments for NBP in primary care. Poor coordination and long wait times negatively affect patient experiences. Effective NBP management should address daily functioning, work participation, and comorbidities.
