Among 130 consecutive patients admitted with verified myocardial infarction, 13 fulfilled the criteria for recurrent infarction or extension of infarction in the course of the first period of hospitalization. 2 patients had genuine recurrent infarction and 11 had extension of infarction as assessed by the conventional clinical criteria together with ECG and enzyme recordings. These early ischemic complications occurred between the 6th - 18th days after the primary infarction. The mortality was 31%, particularly in patients with primary anterior wall infarctions. It appears probable that the risk of recurrent infarction or extension of infarction is increased by treatment with preparations with positive inotropic effects in the early phase of acute myocardial infarction.
