Stratifying elderly with multimorbidity to integrated care: a scoping review

Abstract
Background: Elderly with multimorbidity experience high healthcare utilization, high rates of hospitalization, and reduced life expectancy. In addition, transition across healthcare sectors are fragmented with increased treatment complexity. In order to ensure balanced utilization of healthcare services, stratification is required to select patients to integrated care interventions through cross-sectoral collaboration and care. The aim of this review was to map existing literature on stratification tools used for elderly with multimorbidity in need of integrated care, defined as cross-sectoral collaboration and care. Methods: MEDLINE, Embase, PsycINFO, Cochrane Library and CINAHL was searched. Two authors screened all articles by title-abstract and full-text using pre-specified eligibility criteria. Knowledge users were consulted through consultation exercises throughout the review process. Results: 12.071 studies were identified. 3 studies were included for analysis. Concept aggregations of excluded studies revealed heterogeneity and a lack of reporting related to the population, intervention and context. Conclusion: Stratification tools for elderly with multimorbidity in need of integrated care is lacking. Future research should focus on developing stratification.
